| This year, all release forms will be
filled out together at the Ohio
Youth Rugby website. This information must be filled out BEFORE
a player can participate in any practice. Please be sure to fill out
the form completely and have a parent agree to the terms and conditions of
the sign-up.
Do not pay the dues via the website. We will
collect the dues as a team and then send them to Ohio Youth Rugby.

- This form is used to give new and prospective players and idea of what rugby is and what
it involves.
